CHANGELOG – ARGGT Disconnect

• Complete module restructure and rename from "ARGGT_DisconnectRagdoll" to "ARGGT_Disconnect".
• Removed legacy GM component:
  - ARG_DisconnectUnconscious_GMComponent.c
• Introduced new modular architecture under ARGGT_Disconnect:
  - ARGGTDisconnect_BaseGameMode.c (custom GameMode integration)
  - ARGGTDisconnect_DamageManager.c (custom damage handling logic)
  - ARGGTDisconnect_InspectCasualtyWidget.c (custom Inspect Casualty UI integration)
• Added custom UI layout for Inspect Casualty:
  - InspectCasualtyMenu.layout (+ meta file)
• Added new project configuration:
  - addon.gproj
  - resourceDatabase.rdb
• Updated thumbnail and mod structure to align with ARGGT ecosystem standards.
• Transition from isolated ragdoll-only logic to a broader disconnect handling system integrated with damage and UI systems.
